APL-NDPP supports GPs to prevent type 2 diabetes in their at-risk patients.

Using our APL-NDPP tool, GP practice teams can quickly list patients registered with them who are eligible for behaviour change programmes through the NHS Diabetes Prevention Programme, and can prepare a prioritised list of people to invite.

How it works

Practices run a single search in their clinical system and import the results into the tool to:

List all eligible patients

List all patients at risk from type-2 diabetes who may be eligible for NDPP behaviour change programmes. Export a filtered list to send invitations for referral.

Prioritise

Rank patients according to the number and importance of their risk factors, and filter according to whether they have already been referred or not or have declined referral.

Tailor their approach

See relevant details for each patient in the list without leaving the tool - including age, vulnerabilities and language spoken.

APL-NDPP supports an individualised approach to patient referral. The ‘interpreter needed’ and 'language' fields allow for coordination of language-based referrals across an area. Our Diabetes Prevention Programme provider can arrange language-specific groups if they receive referrals within a similar timeframe, and this has the potential to help address inequity in access. This is particularly important for diet and lifestyle education programmes, where cultural sensitivity can help increase the likelihood of behaviour change.

How to access

APL-NDPP is available free of charge to GP practice teams in the North East London NHS region. If you have not yet downloaded this tool, or need support using it, please contact your local CEG facilitator.
