Our resources support the NHS England 'Reasonable Adjustment Flag' system by helping GP teams to record Reasonable Adjustments for their patients
The Reasonable Adjustment Digital Flag is an NHS England system for health and care professionals to record, share and view details of Reasonable Adjustments across the NHS for patients with a disability. GP practices are required to have a process in place for recording:
We have developed a template to help practices in North East London capture this information, and a pop-up alert protocol that indicates when a Reasonable Adjustment has been recorded so those patients can be better supported. The resources are also available in other London regions who have an agreement with us in place.
Our Reasonable Adjustment Flag template is available for EMIS and SystmOne. It contains all relevant codes that have been specified by NHS England. This helps practices to standardise the way they record information about Reasonable Adjustments in the patient record. See the template in action in our demonstration video below:

Our pop-up alert protocol triggers when a healthcare professional opens a patient’s record for whom a Reasonable Adjustment code has been added. This makes the flag actionable, supporting GP practice teams to make adjustments for those patients.
Our Reasonable Adjustment Flag template and protocol are currently available in NHS North East London and other London regions who have an agreement with us in place.
