Supporting people with multiple long-term conditions and complexity

The focus on this theme is around our work on:

·       Multimorbidity and frailty

·       Polypharmacy

·       Diabetes and cardiometabolic disease

·       Exploring and intervening in generalist medical practice 

·       Self-management and non-pharmacological interventions for long term conditions, especially common respiratory conditions

·       Cancer survivorship

Areas of work include supporting patients to self-manage their conditions more effectively, tackling inappropriate polypharmacy and redesigning health care delivery.
